If you are having nipple pain during pumping, or you are not able to get a letdown, it may be that a larger breast shield will help. To determine your sizing, hold a half-dollar over your erect nipple (the way it looks when your baby is nursing). If your erect nipple is the same size as the half-dollar, order the extra-large shields. If your nipples are closer to the size of a quarter, consider Medela's large breast shields. This method is only a rough indication of your sizing; the correct breast shield size is also affected by the milk producing structure that lies under the skin.
